PLEASE INITIAL, I am responsible for knowing and complying with the ordinances of the city in which I reside. I will not exceed the amount of animals I am authorized to house based on the city guidelines, or guidelines outlines by a lease.
PLEASE INITIAL All foster animals shall be provided with proper food, fresh water and proper shelter. The foster animal will be housed indoors, and in no situation will be left outside on a porch. The animal will be treated humanely by all members of the foster household.
PLEASE INITIAL The Foster Volunteer will not dispose of the animal in any way. The foster volunteer cannot give the animal away to another animal humane society or person, and will not abandon the animal in any way.
PLEASE INITIAL The Foster Volunteer agrees that all adopted cats be spayed or neutered. If not currently spayed/neutered, the animal being fostered is either ill or too young to undergo a sterilization procedure. The foster volunteer agrees NOT to use this animal for any type of breeding purpose.
PLEASE INITIAL If a representative of Amsterdam Kitten Adoption determines that the animal is not being cared for properly by the foster volunteer; Amsterdam Kitten Adoption has the right to take possession of the animal and cancel this agreement
PLEASE INITIAL The Foster Volunteer accepts any/all liability for any damage caused by this animal while it is under his/her care.
PLEASE INITIAL, I do understand that by participating in Amsterdam Kitten Adoption Foster Program I may receive an animal that has an undetected contagious medical condition. I understand that my personal pets may contract illnesses passed on from the foster animal and will in no way hold Amsterdam Kitten Adoption liable for any conditions my personal pet acquire. I understand that Amsterdam Kitten Adoption recommends that I keep my personal animal(s) separated from the foster animal(s) for a minimum of 10 days as a precaution.
PLEASE INITIAL The Foster Volunteer acknowledges that if any visits to a private veterinary clinic are made; they should first call Amsterdam Kitten Adoption. Any treatment received outside of Amsterdam Kitten Adoption program will be the financial responsibility of the foster volunteer.
PLEASE INITIAL As a rescue in the state of New York, we must maintain registration with the Department of Agriculture and Markets. A.K.A. must submit all foster caregivers name, address, telephone number and description of facilities to the Department of Agriculture and Markets. Additionally, the Department of Agriculture and Markets requires consent from A.K.A. granting them free entry to registered premises, buildings and offices. This includes foster homes, as they are considered an extension of our facility
PLEASE INITIAL, I acknowledge that I have read, understand and will adhere to all the terms, conditions, policies and procedures of Amsterdam Kitten Adoption Foster Program. I understand that if at any time I cannot fulfill these terms, conditions, policies and procedures of Amsterdam Kitten Adoption Foster Program I will, by mutual agreement end my tenure as a Foster Volunteer.
