Fill out one form for each animal you are requesting an appointment for.

**Due to High Demand for our low-cost Spay/Neuter Clinics, our current wait time can be up to 3 months for cats and 4-5 months for dogs. Please be aware of this before putting your pets on our list. If you schedule your pet somewhere else, notify us ASAP so we can remove you from our list.

(NOTE: We do not offer spay/neuter services for dogs over 75 pounds)

If so, proof MUST be shown the day of the clinic. You may also upload your document(s) here. Low income pricing is available to Athens County residents ONLY. (Ohio Works First schedule letter, Ohio Direction Card, WIC card, Section 8 housing voucher, Ohio Medicaid Card, Veterans Benefits Administration letter, Supplemental Security Income benefits letter, Disability benefits letter from the Social Security, or Current tax return showing that your family income does not exceed 150% of poverty line)

You MUST show proof of financial assistance and Athens County residency on the day of your appointment to qualify for the reduced rates, otherwise you will be charged the "standard rate" or your appointment will be cancelled.

Our clinics are held at 1 W Main St in Jacksonville, OH. This is approximately 15-20 min from Athens. The building is right on Rt 13 in Jacksonville, corner of Sixth St. This is our only location.
