APES Shelter and Rescue waiting-list request form for owner or keeper surrender enquiries. Designed to create or update the person record and create a waiting-list entry before redirecting to the APES Animal Surrender Intake Pre-Assessment form. This form also records service-contact permission and GDPR contact preferences for the resulting person record.

APES Animal Surrender Request

Purpose of this form: this is the first step for requesting help from APES Shelter and Rescue where you are seeking to surrender an animal. It enables APES CIC to create or update the owner record and add the animal to the ASM waiting list for review.

Important: completing this form does not guarantee that APES Shelter and Rescue can accept your animal, and it does not place you as high priority on our waiting list.

Our waiting list helps us manage intake requests fairly against available space, species-specific accommodation, quarantine capacity, welfare risk, legal considerations, staffing, veterinary capacity and current animals already in care. Entries may be reviewed, ranked and updated by APES staff. The waiting list is not a booking system and does not create an obligation for APES CIC to accept the animal.

Next step: after this request is submitted you should be redirected to the APES Animal Surrender Intake Pre-Assessment form. That second form asks the detailed husbandry, welfare, health and evidence questions needed for assessment.

Waiting-list creation: APES uses this form to record your contact details and the animal's basic waiting-list information in Animal Shelter Manager. The follow-up pre-assessment form collects the detailed welfare and husbandry assessment.

Data protection and contact preferences

APES CIC will use the information you provide to assess your surrender request, create or update relevant ASM records, manage the waiting list, contact you about this request, and protect animal welfare.

1. Before you begin

This short request form records the owner, basic animal details and reason for surrender. Detailed welfare, husbandry, medical history, enclosure information and photo evidence will be collected in the redirected Intake Pre-Assessment form.

You must understand this before submitting.
This is required so APES CIC can assess welfare, husbandry, accommodation and risk.
APES CIC may need evidence of ownership or authority before progressing a surrender.
Do not include unnecessary sensitive personal information.

2. Owner / keeper details

ASM needs a surname/last name to create or match a person record.
Please include house number/name and street.
Use a number APES CIC can contact you on about this request.
Optional. Provide another number if APES may use it when your main mobile is unavailable.
Used for confirmation emails and surrender-request contact.

3. Animal details for waiting-list entry

Keep this section factual and brief. The redirected pre-assessment form will ask for detailed care, health, enclosure and photo-evidence information.

If unnamed, enter Unknown or Not named.
ASM waiting-list recognised animal type. Select the closest APES category.
ASM waiting-list recognised species field. Use the closest species or species group if exact species is unknown.
ASM waiting-list recognised breed1 field. For exotics, use morph, phase, locality, breed or common descriptive type where relevant.
Main colour, markings, pattern or distinguishing visible features.
ASM reads the first letter for male/female where applicable. Use Mixed group, Unknown or Not applicable where sex cannot be confirmed.
Approximate age in years. Fractions are acceptable, for example 0.5 for six months. If unknown, leave blank and select an age group below.
ASM waiting-list recognised age group. Choose the best fit if exact age is unknown.
Use only if the date of birth is known or can be reasonably evidenced. Otherwise use approximate age and age group.
Only add details you already have available.
ASM waiting-list recognised neutered/spayed field. Use Not applicable for species where this is not relevant.
Optional. Enter a number only in the ASM system units if known. Do not include units or words.
If more than one, give the number and explain below.
Names/species/count only. Full details can be given in the pre-assessment form.

4. Photographic evidence

Please upload clear photographs where it is safe and reasonable to do so. Do not handle, restrain or disturb the animal purely to obtain a photograph. Photographs help APES Shelter and Rescue assess the waiting-list request, triage welfare concerns and plan any follow-up.

If the animal has visible injuries, visible health concerns, or concerns linked to housing or keeping conditions, please use the relevant evidence upload fields below.

Upload a clear photo showing the whole animal where possible.
Optional additional photo, for example another angle, close-up identification feature or group companion.
Select Yes if there are wounds, swelling, bleeding, limb issues, shell/beak damage, burns, trauma or similar visible concerns.
Include where the injury is on the body and whether veterinary advice has already been sought.
Upload a close photo if safe to obtain. Do not cause stress, pain or risk to the animal to obtain this.
Optional additional injury photo.
Examples include abnormal posture, weight loss, swelling, discharge, retained shed, mites, respiratory signs, lethargy or other visible concerns.
Describe what you can see and when it was first noticed.
Upload a clear photo of the visible health concern if safe to obtain.
Optional additional health issue photo.
Select Yes or Unsure if the surrender is linked to unsuitable housing, heating, lighting, humidity, hygiene, overcrowding, equipment failure, escape risk or similar issues.
Keep this brief. The detailed husbandry assessment will be completed in the next pre-assessment form.
Upload a photo of the enclosure, equipment, conditions or issue if safe and appropriate.
Optional additional photo of the housing or keeping issue.

5. Surrender request summary

Leave as Surrender for this service.
ASM waiting-list recognised reason field. Choose the closest primary reason; give details in the next box.
Keep this concise. Detailed background can be added in the pre-assessment form.
Required by ASM for waiting-list creation. Summarise the animal, reason for surrender, urgency and key welfare points.
This helps APES triage, but does not guarantee priority or acceptance.
If there is immediate suffering or danger, seek urgent veterinary help or appropriate emergency support now.
Do not provide a full medical history here. The pre-assessment form will ask for welfare and health detail.
This does not decide whether APES CIC can help, but helps us plan resources.
Optional additional animal notes. This is separate from the mandatory waiting-list description and helps avoid losing context during incoming-form processing.
Optional person/contact notes for APES staff, for example communication needs or access notes.

6. Declarations, data protection and contact preferences

This is service-related contact needed to progress your surrender request. It is separate from wider communication preferences.
Required so APES CIC can process your surrender request, manage the waiting list and contact you about the animal. This is not marketing consent.
Please ensure contact, animal and welfare information is accurate. Incorrect information may delay assessment or affect welfare decisions.
ASM-recognised GDPR contact preference field. Choose the communication routes APES CIC may use for wider contact preferences. Service-related contact about this request may still be required.
ASM-recognised field. If set to Yes, the created person record should be excluded from bulk email where ASM applies this flag.
Optional. For example, accessibility needs, communication preferences, or a request to update contact details.
