A few important things help us keep our animals, volunteers, staff, and community safe.

I understand that animal behavior can sometimes be unpredictable. *
I agree to follow HCHS safety procedures and staff instructions while volunteering. *
I agree to treat animals, staff, volunteers, visitors, and members of the public respectfully. *
I understand that I may only perform volunteer tasks that I have been approved or trained to perform. *
I understand that submitting this application does not automatically guarantee acceptance into the HCHS volunteer program. *
VOLUNTEER LIABILITY WAIVER
HUNTINGTON COUNTY HUMANE SOCIETY
Waiver, Release of Liability &
Acknowledgment of Risk
I hereby acknowledge that I have voluntarily applied
to assist the Huntington County Humane Society
("HCHS") in its work.
As lawful consideration for being permitted by HCHS
to participate in volunteer service at or on behalf
of HCHS, I hereby fully and forever release,
discharge, acquit, and exonerate HCHS, its affiliates,
and their employees, officers, directors, volunteers,
interns, and all others acting on their behalf from
any and all actions, causes of action, claims, or
liabilities that I have or may have in the future,
whether known or unknown, arising out of or related
in any way to my volunteer duties.
ACKNOWLEDGMENT OF RISKS
I understand there are certain risks and dangers
associated with working with wild, feral, and
domesticated animals, including but not limited to:
bites, scratches, zoonotic diseases (diseases
transmitted from animals to humans), and allergic
reactions. I also understand there may be risks
associated with exposure to chemical cleaning
products while performing my volunteer duties.
I fully understand these risks and dangers.
I fully assume all risks involved, acknowledge that
they are acceptable to me, and agree to use my best
judgment in undertaking these activities. I further
agree to follow all rules, policies, procedures, and
safety instructions provided by HCHS employees and
volunteers authorized to act in a supervisory capacity.
I waive and release HCHS, its affiliates, their
employees, officers, directors, volunteers, interns,
and all others acting on their behalf from any claim
for personal injury, property damage, or death that
may arise from my participation in the HCHS
volunteer program.
IMPORTANT ACKNOWLEDGMENT
I HAVE CAREFULLY READ THIS WAIVER AND RELEASE
AND FULLY UNDERSTAND ITS CONTENTS.
I understand that this document is intended
to be a release of liability and a contract
between myself and the Huntington County Humane
Society. I acknowledge that I am accepting
these terms voluntarily and of my own free will.
PARENT OR LEGAL GUARDIAN ACKNOWLEDGMENT
If I am under 18 years of age at the time of
registration, my parent and/or legal guardian
has completely reviewed this Waiver and Release,
understands and consents to its terms, and
authorizes my participation in volunteer activities
with HCHS.
