Last Litter Program Contract & Surrender Form

SCHEDULE OF MEDICATION ASSESSMENT & VETTING

Initial Assessment: KD/PD will set an appointment for the owner to bring in the puppies / kittens for an inital assessment. If KD/PD agrees to accept the puppies / kittens into the program, our team will administer the appropriate vaccines and/or other treatments for the puppies / kittens appropriate for their age and current health.

KD/PD’s vaccination schedule is as follows:

4 weeks of age: Dewormer and initial intake information
6 weeks of age: Dewormer and Booster Vaccine #1
9 weeks of age: Dewormer and Booster Vaccine #2

Intake: Puppies can be spayed/neutered when they are at least eight (8) weeks of age, weigh at least three (3) pounds and healthy; kittens can be spayed/neutered when they are at least eight (8) weeks of age, weigh at least two (2) pounds and healthy. At that point, their surgery date will be scheduled and after their surgery they will stay at KD/PD for adoption.

Surgery Schedule for Mother: Approximately two (2) weeks later, when the mother’s milk has dried up, KD/PD will coordinate with the owner and schedule a convenient surgery date to spay the mother dog or cat.

There is NO cost to the owners for any of the vaccines, deworming or spay/neuter costs for the mother or puppies/kittens provided the owner meets the provisions of this agreement.
