Children under 16 years of age may only volunteer under the direct supervision of a parent/guardian at all times.

Children 16-17 years of age may volunteer without a parent/guardian present.

I agree to abide by the rules and regulations set forth in this agreement and as may otherwise be made known to me in order for the child/children listed below to volunteer with and for TNR A Better Chance. This agreement applies to all volunteer opportunities offered to minors by TNR A Better Chance at all of its locations, events, and programs.

As the parent/guardian for the listed child/children, I want my child to have the opportunity to work with the animals. I understand that there are dangers involved in working with animals. These dangers may include, but are not necessarily limited to, being bitten, kicked, stepped on, clawed, scratched, tripped, allergic reactions, and exposure to zoonotic diseases (e.g., cat scratch fever). I understand that these dangers may result in severe injury, scarring or other permanent disfigurement, or even death of the child, and knowingly assume the risks of allowing my child to participate in the volunteer opportunity.

I represent and agree that the information listed below regarding the age(s) of the child/children is true and correct. I understand that there are age restrictions applicable to certain volunteer opportunities and various animal areas at TNR A Better Chance, and agree that I will abide by the age restrictions set forth in the description for each volunteer opportunity. I understand that the minimum age restrictions are guidelines and that even if my child meets the minimum age requirement, I will use my judgment as to whether my child has the maturity necessary to follow directions and otherwise safely participate in the volunteer opportunity. I will inform the staff of TNR A Better Chance if my child has any known health conditions or behavioral issues that may make a volunteer opportunity inappropriate.

I understand that my child may be required to remain under my custody and supervision (or the custody of another adult designated by me below) during his/her participation in the volunteer opportunity. I agree to obey all safety rules, regulations, and directions of the staff of TNR A Better Chance. I acknowledge that TNR A Better Chance has the right to revoke volunteer privileges if these rules and regulations are not followed or if there are other concerns regarding the child’s ability to safely participate in the volunteer opportunity (including considering the safety of the child, animals, staff, and volunteers).

In consideration for the child/children identified below being permitted by TNR A Better Chance to participate in the volunteer opportunity, I agree to the following waiver, release and indemnification:

The undersigned parent or guardian of the minor(s) below, for himself/herself and on behalf of said minor child/ children, hereby agrees to release, save and hold harmless, indemnify and forever defend TNR A Better Chance, its directors, officers, agents, employees and volunteers, from and against any and all claims, actions, causes of action, demands, liabilities, suits, expenses (including reasonable attorney fees) and negligence made or brought by said minor(s) or by anyone on behalf of said minor(s), as a result of said minor’s participation in the activities with and for TNR A Better Chance. The undersigned parent or guardian of the minor(s) below, for himself/herself and on behalf of said minor(s), further agrees not to sue TNR A Better Chance as a result of any injury, paralysis or death that said minor(s) suffers in connection with his/her participation in the activities of TNR A Better Chance.

I agree that in the event any part of this Agreement is held to be invalid, the remaining parts shall be enforced to the full extent permitted under applicable law.

CONSENT TO MEDICAL TREATMENT

In the event that my child is permitted to volunteer without my accompaniment or is under the supervision of an adult designated below, I hereby consent to the administration of whatever x-ray, examination, anesthetic, medical, surgical or dental diagnosis or treatment and hospital care may be deemed necessary for my child’s safety and welfare in the event of any illness or injury incurred while participating in the volunteer opportunity.

I understand that any resulting expenses or charges are my responsibility as the child’s parent/guardian and that TNR A Better Chance is not financially responsible for such expenses or charges.

INFORMATION REGARDING CHILDREN

PERMISSION FOR ADULT OTHER THAN PARENT/GUARDIAN TO ACCOMPANY CHILD/CHILDREN (IF APPLICABLE)

As the parent/guardian of the above listed child/children, I hereby give permission for the following adult(s) to accompany the child/children. The designated adults have agreed to abide by all the rules and regulations set forth in this document. I acknowledge that each adult will be required to create a separate profile to volunteer at TNR A Better Chance.

I have read and understand this agreement and am of age and otherwise legally competent to sign it. I am signing this document voluntarily with the understanding that I am giving up important legal rights.
