Foster-to-Adopt Contract
I am willing to take immediate custody of the animal(s) and serve in the capacity of a foster-to-adopt home for Kitten Division until all required medical treatment is done. Once the animal has completed Kitten Division’s required medical treatment, the adoption will be finalized. I am able to bring the animal(s) to Kitten Division at a mutually agreed upon date and time for the animal’s spay/neuter surgery. *
I understand that the adoption fee must be paid at the time of this application and is non-refundable. In exchange for the adoption fees, Kitten Division covers the expense of spay/neuter, age appropriate vaccinations, microchip, flea and worm treatment. All procedures are done at Kitten Division. Any expenses I incur that were not authorized by Kitten Division will NOT be reimbursed. *
I understand that Kitten Division is the sole and rightful owner of the animal(s) in my foster-to-adopt care until such time that the adoption is finalized. All decisions regarding any and all foster-to-adopt pets are to be made by Kitten Division Foster Program Team Member until the adoption has been finalized. *
I understand that I do not have the authority to give, transfer, adopt out, or otherwise give custody of any foster-to-adopt animal(s) to any other person, or organization until my adoption of the animal(s) has been finalized. *
I agree that Canine's in foster-to-adopt care will be provided the following: Reasonable exercise in a fenced yard, dog run, or on a leash; Chaining and/or tethering will NOT be used as a primary means of confinement; The foster-to-adopt pet(s) shall be under the immediate control and protection of the foster-to-adopt parent(s) at all times and will not be permitted to roam outside of the fenced area. *
I agree to keep the Foster Program Team updated on the health, behavior, and temperament of my foster-to-adopt pet(s), and agree to report any medical concerns to Kitten Division immediately. *
I understand that I must consult with the Foster Program Team, before taking the foster-to-adopt pet(s) to an unauthorized veterinary practice. I understand that if I choose to take my foster-to-adopt pet(s) to any unauthorized veterinary practice, it will be at my own cost, and I will NOT be reimbursed by Kitten Division. *
I understand that if any foster-to-adopt pet(s) is harmed due to my negligence, I will be held responsible for the veterinary bills, surgery costs, and/or medications. *
I agree to supervise any and all interactions that occur between my foster-to-adopt pet(s) and my personal pets, as well as any and all interactions that occur between my foster-to-adopt pet and any children, regardless of whether the child(ren) reside in my residence or not. *
I agree to hold Kitten Division harmless from any direct, indirect, or consequential damages arising out of this foster-to-adopt agreement. I hereby accept this animal and assume all the risks and responsibility associated with this animal; including but not limited to bites, and fully and completely release, indemnify and hold harmless, Kitten Division, its owners, directors, volunteers, and agents from any claim, cause, action, or liability, of any sort or nature, whether, known or unknown, directly or indirectly arising out of, or in connection with, this animal. *
