I have read the foregoing and certify that the answers I have given are complete, true and not misleading in any way. I am also aware that BVAR cannot guarantee the health and temperament of the animal. I am also aware that this is NOT an adoption agreement, merely an application and my application will be reviewed to determine suitability to adopt the animal in question. My application form will be deemed incomplete until I have supplied BVAR via email copies of vet work for my existing pets, landlord approval for a pet and photos of my yard. *
