Care: I hereby agree to care for the above-described pet in a humane and responsible manner and to provide it with clean and adequate shelter, food, water and veterinary care. I further agree that said pet shall reside inside my home and shall not be allowed to roam freely. Identification: I hereby agree to register my pet’s microchip. I agree to always keep my pet’s microchip contact information up to date with the registration company. Adoption Fee: I understand that the adoption fee helps to cover the current owner’s expense of vetting, raising & caring for the pet and is not refundable. Veterinary Care: We strongly recommend you have said pet examined by a licensed veterinarian within 10 days of adoption and provide medical treatment as needed. I agree to keep all vaccinations current, including Rabies. I agree to ensure that the pet receives preventative medication to protect against pests. Any other veterinary problems that may arise, including serious illness requiring emergency care or surgery, is my responsibility as well as all related costs. I agree to NEVER declaw above-described pet. Warranty: I agree that S.C.A.R.F. and it’s volunteers cannot guarantee the health of an adopted pet. Even though a pet may seem healthy, there is a chance it could be incubating a disease without showing signs. There is no warranty as to the temperament or physical condition of said pet. I agree that there is an adjustment period for the adopted pet and my existing pets. See “Adopting a Cat 101” If for any reason I decide to give this adopted pet up, I agree to contact S.C.A.R.F.
