Upon completion of this form, your information will be reviewed, and either approved or denied. Completion of this form is in no way an agreement with Adore Pet Rescue to take custody of the animal. Once the application for surrender has been received and reviewed, we will reach out to you about accepting the pet. This will allow us to assess what training, lifestyle and best foster placement is needed for the best success of the animal. Any donation you could make would be used for the care and needs of your surrendered pet.
