Dog Day Out Volunteer Application

Thank you for having interest in our Dog Day Out program. The mission of this program is to help us get a better understanding of the dogs while they are here at the shelter. Please read through and check beside each requirement listed below. Then, complete the volunteer information portion at the bottom of this form.

*	All volunteers agree to be dressed in a presentable manner while in care of said dog. This includes wearing proper foot wear. Please no sandals or open-toed shoes. If the volunteer is not dressed accordingly, the shelter employee may refuse the volunteer the right to volunteer.
*	Volunteers agree to arrive for their committed time to take said dog out for the day and to return them to the Nevada Animal Shelter by the agreed upon time.
*	Volunteers may only take one dog away from the shelter at a time. The volunteer also agrees to keep said dog on a leash at all times, unless in a properly secured area such as a fenced in backyard.
*	Volunteers may NOT interact said dog with the volunteer’s own animals or any other animals at any time. This should be a one-on-one experience for the volunteer and dog.
*	Volunteers agree that they are NOT allowed to give said dog away to another person or allow other people to take care of said dog while in their care.
*	Approved volunteers must be 18 years of age with a current form of I.D.

*	FOR THE SOLE CONSIDERATION of Permission of the City of Nevada, Missouri, allowing me to volunteer at the Nevada Animal Shelter for the Dog Day Out program, I, the undersigned hereby release and forever discharge the City of Nevada, its agents and employees, jointly, separately, and individually therein and expressly deny liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, and particularly on account of all injuries, known or unknown to both person and property, which have resulted or may in the future develop from volunteering at the Nevada Animal Shelter and for the Dog Day Out program.
*	Undersigned hereby declares that the terms of this release have been completely read and are fully understood and voluntarily accepted for the purpose of volunteer for the Dog Day Out program at the Nevada Animal Shelter and for the settlement of any and all claims, disputed or otherwise, on account of the injuries and damages above mentioned, and for the express purpose of precluding forever any claims arising out of the aforesaid volunteering at the Nevada Animal Shelter. The Nevada Animal Shelter reserves the right to refuse any and all persons from participating in this program. If at any time the volunteer breaks any of the agreed upon terms, the Nevada Animal Shelter reserves the right to terminate this contract for volunteering for the Dog Day Out program and any other volunteer programs with the Nevada Animal Shelter.
