*	To increase my chance of finding my pet, I will continue to take reasonable efforts to find my pet, including contacting the shelter regularly and checking their website/social media.
*	Filling out this form does not remove any reclaim fees I’m responsible for if my pet were to end up at DHS.
*	DHS follows Indiana’s 3-day stray hold. If your pet is at DHS for longer than 3 days they may be put up for adoption.
