Thank you for your interest in adopting a dog from Dog Star Rescue. Adopters must be at least 21 years of age to adopt. Please fill out this application in full so that we can process your application.

Please Note: Submitting an application does NOT guarantee you will be matched to the dog you applied for. We reserve the right to deny anyone to adopt a dog without explanation. Dog Star Rescue is thorough in our placement decisions to ensure that our dogs are placed in their adoptive homes successfully.

Applications are usually processed within 3 business days. However, it can take longer depending on verification of your veterinarian/landlord/references, as well as if you apply after Thursday and before Sunday. Please note that applicants must call their veterinarian upon submission of the application to give their permission for the veterinarian to share information with us. Please email adopt@dogstarrescue.org once you have given your vet permission. Also, all sections of the application must be complete. If your release is not given to your vet, or you left parts of the application blank, then your application will NOT be processed.

General Disclosures: 1. The Dog Star adoption fee covers all vetting costs, including pull fees charged by shelters, spay/neuter costs, vaccinations including bordetella, distemper/parvo, rabies, fecal and heartworm screening, heartworm and flea preventative, interstate health certificate and transportation fees. 2. After filling out the Application, please call your veterinarian to give Dog Star permission to contact your vet. 3. This Application shall be governed by, and construed in accordance with, the laws of the State of Connecticut, irrespective of the laws that might otherwise govern under applicable principles of conflicts of law.

I understand that Dog Star Rescue has the right to deny any application, and even if I am applying for a certain canine I understand that canines are adopted to the best home and not first come, first serve basis. My application may still be approved but may be better suited for another canine. *
I understand Dog Star Rescue cannot guarantee the health or temperament of any canine, as many canines come in with unknown histories. There are traits could change upon adoption and I take responsibility to care and address these issues if they do arise. *
I agree to make a veterinary appointment to introduce our/my canine to our/my veterinarian, and to address any medical issues or concerns immediately. *
I also understand that anything could happen in life, and if I find that I am no longer able to care for my adopted canine, I agree to contact DSR by email (contactus@dogstarrescue.org) to discuss return/or need assistance with my adopted canine. DSRS will be here to discuss any concerns or issues that you may be having with your newly adopted family member. *
