Dog Adoption Application

Thank you for your interest in adopting a shelter dog from I Heart Dogs Rescue! The following application is for adoption ONLY. If you are interested in FOSTERING a dog, please fill out our FOSTER APPLICATION here. Please note the following requirements:

Our standard adoption radius is 100 miles from our shelter located in Warren, MI.
You must be 21 years old to adopt.
Your homeowner must consent to the adoption of the animal. If you rent, you will need to provide a copy of the pet policy in your lease. It can be attached to your application.
All members of the household, including children, must meet our dogs.
If you presently have a dog(s), we will facilitate dog introductions at our shelter.
We perform vet checks to confirm spay/neuter, vaccines, and heartworm prevention.
