Thank you for considering fostering through I Heart Dogs Rescue! Please complete the following adoption application in it's entirety. Please note that any falsification of information will cause your application to be rejected. Incomplete applications will not be processed. Before beginning, please note the following requirements:
Prior to taking your foster dog home, we must see a driver’s license or a state-issued ID with your current address showing you are at least 21 years old.
The owner of your home must consent to the adoption of the animal. If you rent, we will need to see a copy of the pet policy in your lease. This can be attached to your application or you can show a copy prior to adoption. Please note that we only send dogs home with people living within a 50 mile radius of our shelter located in Warren, MI.
All members of the household, including children, must meet and approve of your foster pick.
If you presently have a dog(s), we invite you to bring them with you as all dogs must participate in a meet and greet at the Haven.
We will require contact information for your veterinarian if you have or have had other pets in the last 5 years. We may contact your vet to ensure that your pets are/were kept up-to-date on their vaccinations. This is for the safety of your own pet as well as the foster dog you are welcoming you’re your home. If your vet requires your approval to release that information to us, please call them ahead of time to expedite the process.
Your signature on the application is required. You can use your mouse to sign on the line at the very end of the application. If you are filling the application out on your smartphone or tablet, you should be able to sign using your finger or a stylus.
Once we receive your foster application, you can expect to...
• Receive a confirmation email
You should receive a confirmation message to the email address listed on your application. So long as you have received this email, you can be confident that we have received your application!
• Speak with our Foster Coordinator
If we think we have a dog that would fit in your home, our Foster Coordinator will be in touch to ask additional questions and discuss the animal or animals that may be a match for you. All approved applications will move to the next step.
• Meet the Dogs
Meet one or more of our fabulous dogs to identify a match for your family!
• Vet Check
We will contact your veterinarian to ensure that your pets have been vaccinated against any communicable diseases to decrease the risk of any illness developing in your personal pet or the foster dog we are sending to your home.
• Home Visit
When one of our trained volunteers conducts a home check in your home, our goal is to ensure that the environment is safe and secure. We will identify areas that pose a risk with the dog escaping, hazardous materials within the dogs reach, and provide tips to the entire household on the role/s everyone can play in setting the dog up for success. Once the home check has been successfully completed, we will set up a time for you to pick up your foster pet! We will also provide any supplies you may need (crate, food, dishes, toys, leashes, etc) when bringing home your foster pet!
• Timeframe for the application process
Our commitment is to do our very best to complete the entire process within one week; however, we ask for your patience in accommodating unforeseen circumstances that may surface during the process which may prevent us from achieving this goal. You should hear from us regarding your application within 48 hours of submitting it, but it may take up to a week. There is no need to check on the status of your application prior to that.
