1. A separate request form must be submitted for each animal.
2. Once this request has been submitted we will contact you as soon as possible to set up an appointment. Prepayment is required at the time an appointment is made. We accept cash or credit/debit cards.
3. We only provide the services listed below. You will be contacted if additional services are recommended. Payment for any extra services will be required at the time of pick-up.
4. All animals will be given a rabies vaccine unless you can provide proof of a current vaccination (must be a signed rabies certificate).
5. Additional charges may be applied at the time of pick-up for any animal in heat, pregnant or obese.
6. All surgeries will be performed at Franklin County Humane Society, 100 Companion Place, Frankfort, KY 40601
7. NO REFUNDS will be provided for any reason.
8. All animals residing within the city limits of Frankfort are required to have a city license. We are required to collect that fee at the time we schedule the appointment. The cost is $10 for altered animals.

*Sales tax will be added to the services you have selected. A total amount due will be given when the appointment is scheduled.
