I also agree that if I am dishonest on any questions, your application may be denied. I also agree that even though, I submit the application, it is not a guarantee of any animal, or the specific animal applied.

You also agree to let the FCHS do a background check for abusive situations, animals at large, felonies, or any other relevant offence on all members living or staying at the address on your application, your veterinarian, landlord or any other pertinent situations to find the best fit for any animal at the FCHS. You must list all individuals full, legal name on the application. Failure to do so may result in your application being denied. You also understand that by submitting your application, it does not automatically approve your application. We base all applications on the best fit for each animal.
