I HEREBY ASSUME ALL OF RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH VOLUNTEERING AT PUBLIC ANIMAL WELFARE SERVICES, including of not that may arise from negligence or carelessness on the part of the persons or entities being released: dangerous or-defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault I certify I am physically fit, have sufficiently prepared or for participation in this activity, and have not been advised ' to not by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity. I acknowledge this Accident Waiver and Release of Liability will be used by the Public Animal Welfare Services, Floyd County Government, and organizers of the activity in which I may participate, and that it will govern actions and responsibilities at said activity. In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors; administrators, heirs, next of kin, successors, and as follows: (A) I WAIVE, RELEASE, AND DISCHARGE from and all liability; including but not limited to, liability arising from the negligence or fault of the entities or persons released for my death, disability, personal injury, property damage, property theft; or actions of any kind which hereafter occur to me including traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: PUBLIC ANIMAL CONTROL and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers (B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence release or otherwise. I acknowledge that PUBLIC ANIMAL WELFARE SERVICES and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, or failures to act of any party or entity conducting a specific activity on their behalf. I acknowledge that this activity may involve a test of a person’s physical and mental limits and carries it the potential for death, serious injury, and property loss. The risks include, but are not limited to, those caused by animals, facilities, condition of participants, equipment, vehicular traffic, and actions of other people including, but not limited to, participants, volunteers, monitors: and/or producers of the activity. These risks are not only inherent to participants, but are also present for volunteers, I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity. The Accident Waiver and Release of Liability Form shall be construed broadly to provide release and waiver to the maximum extent permissible under applicable law. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN OF MY OWN FREE WILL

Thank you for participating in the P.A.W.S. Volunteer program. Your participation in our program will assist us in getting our animals adopted. Please sign the waiver and return to us. You will need to sign-in each time you come to volunteer. You must be at least 18 to participate. We have restrooms and water fountains in the front lobby. INMATE LABOR IS USED HERE, BY GEORGIA LAW IT IS A FELONY TO GIVE AN INMATE ANY ITEMS 0.C.G.A. 42-5-18 (2014). It shall be unlawful for any person to obtain for, to procure for, or to give to an inmate a gun, pistol, or any other weapon; any intoxicating liquor; amphetamines, biphetamines, or any other hallucinogenic drugs or other drugs, regardless of the amount; any telecommunications device, or any other article or item without the authorization of the warden or superintendent or his or her designee. A person who commits or attempts to commit a violation of this Code section shall be guilty of a felony and, upon conviction thereof, shall be imprisoned for not less than one nor more than five years. Volunteer Dress Code: 1. Volunteers are required to wear appropriate attire. Shoes are required of all volunteers. If a volunteer is wearing clothing not considered appropriate by the front entry staff, the staff shall have the authority to do the following: a. Require the prospective volunteer to change into appropriate attire b. Ask the volunteer to wear a cover up such as a sweater, jacket, or other type of concealing garment during the visit, or c. Terminate the volunteer opportunity entirely, especially if the volunteer refuses to cooperate with staff in this matter. In the event the volunteer opportunity is terminated, the staff shall notify the P.A.W.S. Director. 2. Inappropriate clothing shall include, but is not limited to: a. Dresses or tops with thin straps which expose shoulders or chest area in any manner b. Dresses, skirts, or other similar garments that are more than 2 inches above the knee c. Tube tops or halter tops of any type d. Any type of clothing which reveals the stomach or midriff area e. Any type of clothing that is made of sheer, net, mesh, or transparent (see through) material f. Shorts of any kind or any kind of slacks that are above the knee g. Leggings, jeggings, or any other skin tight pants shall never be worn h. Foundation type undergarments are required i. Any other type of clothing that is suggestive or revealing in any way shall be prohibited
P.A.W.S. PHOTO RELEASE FORM I hereby grant P.A.W.S. permission to use my likeness in a photograph, video, or other digital media ("photo") in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of P.A.W.S. and will not be returned. I hereby irrevocably authorize P.A.W.S. to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless,release, and forever discharge P.A.W.S. from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE.
