Please read each statement below:

1. Ownership and Responsibility
I certify that I have the consent of all members of my household to adopt this cat. Effective upon execution of this Agreement, I accept full legal and financial responsibility for the care and welfare of the adopted cat. I understand that adopting a pet is a lifelong commitment, and I intend to provide a safe, stable, and loving home for the remainder of the cat's natural life. If my living situation, financial circumstances, or personal circumstances change, I agree to make every reasonable effort to continue providing appropriate care or to work with The Furry Paws Foster Inn to identify a responsible alternative placement.

2. Standard of Care
I agree that the cat will be kept primarily as an indoor companion animal and provided with a clean, safe, and sanitary home, fresh food and water daily, appropriate litter boxes, enrichment, and routine veterinary care.

3. Humane Treatment and Declawing
I agree to treat this cat humanely at all times and will never intentionally strike, abuse, neglect, or otherwise mistreat the cat, nor allow another person to do so. I further agree that this cat will not be declawed by any method, including tendonectomy, onychectomy, or similar procedures, except where medically necessary as determined by a licensed veterinarian. I understand that unauthorized declawing may constitute a breach of this Agreement and may subject me to legal remedies available under applicable law.

4. Veterinary Care
I agree to provide routine preventative veterinary care, vaccinations, and prompt medical treatment for illness or injury throughout the cat's lifetime. I understand that if the kitten has not yet received a rabies vaccination because of age or timing of spay/neuter surgery, it is my responsibility to obtain the vaccine when appropriate and provide proof of vaccination to The Furry Paws Foster Inn.
I understand that decisions regarding humane euthanasia should be made in consultation with a licensed veterinarian and only when medically appropriate.

5. Parasite Treatment
I understand that my cat has received one or more broad-spectrum deworming treatments as documented in the medical records provided. However, no deworming medication treats every possible parasite. Should signs of illness or parasites develop after adoption, I agree to seek veterinary evaluation at my own expense.

6. Transfer of Ownership
I agree that this cat will not be sold, used for commercial purposes, abandoned, or transferred for any illegal purpose. If I am ever unable to keep the cat, I agree to contact The Furry Paws Foster Inn before surrendering the cat to another rescue, shelter, animal control agency, or private individual. The Furry Paws Foster Inn will make reasonable efforts to assist in finding an appropriate placement and, when space and resources allow, may accept the cat back into its program. If I wish to transfer ownership to another individual, I agree to notify The Furry Paws Foster Inn so that we may assist in ensuring the cat is placed in a safe and responsible home.

7. Behavioral Concerns
I understand that behavioral issues may arise as the cat adjusts to a new environment. Should significant behavioral concerns develop, I agree to contact The Furry Paws Foster Inn before considering surrendering the cat and will make reasonable efforts to work with my veterinarian and/or a qualified feline behavior professional to address the issue. I acknowledge that kittens adopted without another compatible feline companion may be at increased risk of developing behavioral concerns commonly associated with single kitten syndrome.

8. Animal Welfare
If The Furry Paws Foster Inn has reasonable cause to believe that the cat is being abused, neglected, abandoned, or maintained in violation of applicable animal welfare laws, I understand that the organization may pursue any remedies available under this Agreement or applicable law to protect the welfare of the animal.

9. Future Placement Assistance
I agree that I will not surrender the cat to a municipal shelter or animal control agency without first contacting The Furry Paws Foster Inn. If circumstances require me to rehome the cat, I will first provide The Furry Paws Foster Inn the opportunity to assist with placement.

10. Adoption Fee
I understand that my adoption fee helps offset the medical care and expenses incurred by The Furry Paws Foster Inn and supports future rescue efforts. Adoption fees are non-refundable after seven (7) days following adoption. If the cat is not a suitable match for my household, I agree to contact The Furry Paws Foster Inn to discuss available options before making other arrangements.

11. Spay/Neuter Requirement
If my kitten has not yet been spayed or neutered at the time of adoption, I agree to have the procedure completed by the deadline established by The Furry Paws Foster Inn. If the procedure is to be performed through The Furry Paws Foster Inn's veterinary program, I agree to return the kitten for the scheduled appointment. If I choose to have the procedure performed by my own veterinarian, I understand that it will be at my own expense unless otherwise agreed in writing. I agree to provide documentation of the spay/neuter surgery and rabies vaccination (if age appropriate) within the required timeframe. Failure to comply with this requirement may constitute a breach of this Agreement.

12. Post-Adoption Veterinary Examination
I agree to have my cat examined by a licensed veterinarian within thirty (30) days of adoption, or within thirty (30) days following spay/neuter if the procedure occurs after adoption.

13. Lost Pet Notification
If the adopted cat becomes lost or missing, I agree to promptly notify the microchip company and make reasonable efforts to notify The Furry Paws Foster Inn so that additional recovery resources may be provided.

14. Follow-Up
I understand that The Furry Paws Foster Inn may contact me periodically after adoption to check on the cat's adjustment and welfare. I agree to reasonably cooperate with these follow-up communications.

15. Transition Period
I acknowledge that I have received information regarding the recommended shutdown/integration protocol for newly adopted cats. I understand that following these recommendations will help my cat successfully adjust to its new home and reduce unnecessary stress during the transition period.

16. Enforcement
I understand that this Agreement is intended to protect the welfare of the adopted cat. If I materially violate the terms of this Agreement, The Furry Paws Foster Inn reserves the right to pursue any remedies available under this Agreement and applicable law.

1. Dog Behavior and Disclosure
I understand that every dog is an individual and that behavior may change as the dog adjusts to a new environment. Dogs should always be properly supervised around children and other animals. I acknowledge that the dog's estimated age, breed, and history are based upon the information available at the time of intake and may not be exact.
The Furry Paws Foster Inn has disclosed all known medical and behavioral information available at the time of adoption but cannot guarantee the dog's future temperament, health, age, breed, or behavior.

2. Ownership and Assumption of Responsibility
Effective upon signing this Agreement, I accept full legal and financial responsibility for the adopted dog. I understand that I am responsible for providing lifelong care, including food, shelter, training, licensing, identification, veterinary care, and compliance with all applicable laws.
I agree to release The Furry Paws Foster Inn from liability for injuries or damages caused by the adopted dog after the date of adoption, except where prohibited by applicable law.

3. Lifelong Commitment
I understand that adopting a dog is a lifelong commitment.
If I am no longer able to care for the dog, I agree to contact The Furry Paws Foster Inn before surrendering the dog to another rescue, shelter, animal control agency, or private individual.
The Furry Paws Foster Inn will make reasonable efforts to assist in identifying an appropriate placement and, when space and resources permit, may accept the dog back into its program.

4. Medical Care
I understand that The Furry Paws Foster Inn has provided all known medical records available at the time of adoption.
Because many rescue dogs have unknown medical histories, illnesses or medical conditions may exist that are not apparent during foster care.
I accept responsibility for all future veterinary care and expenses following adoption.

5. Licensing and Identification
I agree to obtain any licenses required by local or state law and ensure that identification tags and registration remain current throughout the dog's life.

6. Humane Care
I agree that my dog will be treated humanely at all times.
The dog will not be left continuously tethered or chained outdoors and will not be subjected to neglect, abuse, or unsafe living conditions.

7. Public Safety
I understand that dogs should never be left unattended in public places.
I agree that my dog will remain leashed whenever required by law and will only be allowed off leash in secure, legally permitted areas.

8. Animal Welfare
If The Furry Paws Foster Inn has reasonable cause to believe the dog is being abused, neglected, abandoned, or maintained in violation of applicable animal welfare laws, I understand that the organization may pursue any remedies available under this Agreement or applicable law to protect the welfare of the animal.

9. Veterinary Care
I agree to have my dog examined by a licensed veterinarian within thirty (30) days of adoption and to provide routine preventative care throughout the dog's lifetime, including vaccinations, parasite prevention, and treatment for illness or injury.

10. Spay/Neuter Requirement
If my adopted dog has not yet been spayed or neutered, I agree to have the procedure completed within the timeframe established by The Furry Paws Foster Inn.
If the procedure is scheduled through the rescue, I agree to return the dog for the appointment.
If I choose to use my own veterinarian, I understand that the procedure will be at my expense unless otherwise agreed in writing. I agree to provide documentation confirming the procedure has been completed.
Failure to comply with this requirement may constitute a breach of this Agreement.

11. Microchip
If my adopted dog is not already microchipped, I agree to have a microchip implanted and provide documentation to The Furry Paws Foster Inn.
If the dog is already microchipped, I agree to promptly register or update the microchip information with my current contact information.

12. Pet Insurance
I understand that veterinary care can be expensive, and The Furry Paws Foster Inn strongly recommends obtaining pet health insurance shortly after adoption. I understand that pre-existing conditions may not be covered if insurance is purchased after medical issues arise.

13. Training and Behavioral Support
I understand that newly adopted dogs require time to adjust to a new home and may experience anxiety, fear, or behavioral challenges during the transition.
I agree to use positive reinforcement training methods and seek assistance from a qualified trainer or behavior professional if significant behavioral issues develop.

Before considering returning the dog for behavioral reasons, I agree to make reasonable efforts to address the issue through appropriate training and consultation.

14. Receipt of Records
I acknowledge that I have had the opportunity to review all available medical records, foster notes, and behavioral information provided by The Furry Paws Foster Inn and have had the opportunity to ask questions before completing this adoption.

15. Transition and Decompression
I acknowledge that I have received information regarding the recommended decompression and integration protocol for newly adopted dogs.
I understand that following these recommendations—including gradual introductions to people, pets, and new environments—will greatly improve my dog's chances of a successful transition into its permanent home.

HEARTWORM POSITIVE DOGS ONLY:

If my adopted dog is heartworm positive, I understand the treatment plan that has been discussed with me and agree to follow the recommended treatment protocol.
If treatment is being coordinated by The Furry Paws Foster Inn, I agree to comply with all scheduled appointments.
If treatment is being completed through my own veterinarian, I agree to provide documentation of treatment progress as requested so that available donated funds, if applicable, may be applied directly toward approved veterinary expenses.
