Describe how your dog behaves when at the vets * Include information on how they behave when entering and waiting outside the consult room, and how they respond to being examined and restrained.

Has your dog ever bitten? * Please note, failure to disclose relevant information which places the dog or a new registered owner in danger or fear for their safety, which has not been disclosed prior to adoption, may result in legal proceedings against you.
Describe any behaviours of concern
Have you ever worked with, or requested support from any kind of professional such as a charity, trainer, behaviourist, groomer, vet, pet sitter, residential training service ect to assist you with any aspect of your dog's care? If yes, please add the professional's name/ website and qualifications if known.
If you are surrendering your dog due to behaviour concerns or a need for support with anything else from finances to food supplies, please let us know what you feel you need and whether you may be able to keep your dog with you should the right help be available.
