Waiver for Cat Care Center Volunteers

READ CAREFULLY - THIS AFFECTS YOUR LEGAL RIGHTS

I agree for myself and (if applicable) for the members of my family, to the following:
I hereby agree that I am providing volunteer services to Life Is Better Rescue, hereinafter referred to as the Rescue, at their Cat Care Center facility located at 3535 South Allison Street in Lakewood, CO, hereinafter referred to as the Care Center.
I acknowledge that my services are provided strictly on a volunteer basis, without any pay or compensation of any kind, and without liability of any nature on behalf of the Rescue. All services to be performed by me are at my own risk.
I recognize that in handling animals and performing other volunteer tasks, there exists a risk of injury to me or my property, including but not limited to physical harm caused by the animals and/or transference of disease by the animals and I hereby expressly assume all such risks.
Furthermore, I give permission for Life Is Better Rescue to broadcast live, make a video and/or audio tape, take photographs, generate computer images, or other images of me in various uses during volunteer activities for the Rescue.
On behalf of myself, my heirs, personal representatives and executors, I hereby release, discharge, indemnify and hold harmless the Rescue, its officers, directors, agents, adoption partners, employees and other volunteers from any and all claims, causes of action, or demands, of any nature or cause, including costs and attorneys fees incurred by the Rescue in connection with the same, based on the damages or injuries which may be incurred or sustained by me or my property or any companion animal I may be in contact with which are in any way connected with my services or as a result of my services for the Rescue, including but not limited to, animal bites, accidents, disease or injuries.
In addition – I agree to comply with the following protocols:
-- I agree to observe and obey all posted rules and warnings, and further agree to follow any oral or written instructions or directions given by identified agents of the Rescue.
-- No animal is to be removed from the Care Center without the explicit authorization of an identified agent of the Rescue.
-- I agree not to remove or destroy any property, supplies or equipment from the Care Center, unless explicitly authorized by an identified agent of the Rescue.
-- In the event that any item is removed without authorization (including animals), I understand the authorities may be contacted and that appropriate charges, including theft, may be pursued by the Rescue.
-- I agree to follow all medication guidelines for the animals and not introduce any outside medications to animals that have not been specifically approved by an identified agent of the Rescue.
-- I agree to keep the animals in the areas they have been designated.
-- I agree to reimburse the rescue for any and all damages caused to the Care Center by the negligence, recklessness or willful actions of me, my family, or any other person whom I bring into the Cat Care Center facility.
-- I understand that if I have questions or experience an emergency, I will follow the established contact guidelines, which have been provided to me by the Rescue. I will also inform an identified agent of the Rescue in the event of any injury within 48 hours of its occurrence.
-- I have received and reviewed contingency plans. I understand and will comply with protocol as set by LIBR for fostering, and will comply.
