Foster Home Agreement

By signing below, I agree to foster until the animal is adopted or is returned to Life is Better Rescue (LIBR).

-- I agree that while the animal is in my care, it will be treated as a household companion, not as an outside pet, and that I will provide her/him with humane care and treatment, including proper food, water, shelter and exercise.

-- I agree to provide emergency medical/veterinary care at an LIBR approved veterinarian for which I will be fully reimbursed upon proof of payment.

-- I agree to safeguard the animal from loss or mishap and to immediately notify LIBR if the animal is lost, and to make every reasonable effort to recover the animal at my own expense.

-- I agree that while the animal is in my care, I won’t foster another animal with another rescue at the same time to avoid any disease transfer or potential aggressive behavior between the two rescue animals.

-- I will NOT give or sell the animal to another person, relative, rescue group, humane association, shelter or pound, or any medical or experimental laboratory or similar organization.

-- I will NOT change or permanently alter the animal’s appearance in any way, shape or form (e.g., cropping, docking).

-- I will immediately notify LIBR in the event of death during the foster care period.

-- I will allow LIBR to make periodic visits as often as it deems it necessary.

-- I will immediately notify LIBR of any change of address or telephone number, or of any plans to move out of the State of Colorado during the foster care period.

-- I will immediately notify LIBR of any change in circumstance which may prevent me from continuing the care detailed in this foster agreement.

-- Should I decide to permanently adopt the animal, I will notify LIBR and, upon approval, will follow established adoption procedures.

-- I understand that the animal has been checked by an LIBR representative and is accurately represented to the best knowledge of LIBR.

-- I agree that by fostering, I am still held to all relevant zoning and animal control codes and ordinances, whether local, county, or state.

-- I have received and reviewed contingency plans. I understand and will comply with protocol as set by LIBR for fostering, and will comply.

In consideration of my participation as a foster care provider, or by volunteering as an animal handler or at any event for which I am a representative of LIBR, I hereby release and discharge, indemnify, defend and hold harmless, the Board of Directors of LIBR, their officers, employees, agents, members and other individuals and entities contributing support, from any liability for damage to or loss of personal property, sickness, injury from whatever sources, legal entanglements, imprisonment, death, loss of money, etc. which may occur as a result of my participation. I agree that my participation is strictly voluntary. I agree to foster the animal at my own risk, and to be solely responsible for any damages to people or property caused by the animal(s). I am fully aware of, understand, and agree to all the terms and conditions of this release and indemnification provision, and that it is binding and enforceable by law.

Furthermore, I give permission for Life Is Better Rescue (LIBR) to broadcast live, make a video and/or audio tape, take photographs, generate computer images, or other images of me in various uses during foster and volunteer activities for LIBR.

I have read and fully understand the terms and conditions of this agreement, and agree that it is not arbitrary and that the provisions herein are common in pet foster care agreements. I further understand that if I fail to comply with any of the terms specified herein, LIBR has the right to reclaim the animal and to pursue any legal channels to do so.
