*	I understand that completing this application does not guarantee I will be allowed to adopt a pet, and that the application is a tool to help Willow Tree Sanctuary determine if this pet and my family are a good fit for each other. I acknowledge that all information provided on this form is true and correct. I understand that any misrepresentations may result in my not being approved and/or removal of the adopted pet from my possession. I also certify that I have never been convicted of animal cruelty or neglect. If I am approved to adopt a pet, I agree to pay a non-refundable adoption fee (with a 2 week trial period) and sign an adoption contract at the time of adoption. I understand that all pets adopted from Willow Tree Sanctuary will be speyed/neutered, heartworm tested, and vaccinated prior to adoption.
*	I understand that Willow Tree Sanctuary are the legal owners of all animals until the full adoption fee is paid and all vet work has been completed.
*	I understand that the adoption fee must be paid in full prior to the animal being sent on trial; or in the case of a foster fail, as soon as the adoption application has been received and approved.
*	I agree to communicate with Willow Tree Sanctuary regarding the progress and welfare of this animal if I have any issues or concerns throughout its lifetime.
*	By checking the box below and accepting an animal into my care, I acknowledge that I am bound by and agree to the above terms and conditions of adopting for Willow Tree Sanctuary. Furthermore I understand that should I be in breach of the above terms and conditions my application will be terminated and I may be held liable (financially or otherwise) for any damage, death, illness or injury caused by or to my animal.
