I understand that I am request Paws Cat Shelter to take permanent ownership of the cats/kittens listed below. If Paws Cat Shelter does take them, they will determine the best; most suitable home for placement. By signing this release form, I affirm to the best of my knowledge that:	The cats/kittens below are stray and homeless. I am returning a previously adopted cat/kitten to Paws. I am permanently surrendering my cat/kitten to Paws.
Circumstances of turnover: Please give us as much information as you can about the cat/kitten, it will help us to find the best adoptive home we can for it.
