Friends United in Rescue, Inc receives multiple relinquishment requests on a daily basis, therefore, you must allow our team ample time to review your request. Once your request has been reviewed and approved, a member of our team will call you at the phone number you provide below. Please make sure your voicemail is in working order as we will only try to contact you once. If you fail to return our call within 48 hours, your request will be removed and we will reach out to the next person in need of relinquishment assistance. Our relinquishment program is on a first come, first serve basis. Do you understand? *

The form below is used to evaluate your request and if your request is approved, the form is used to properly place the cat in a suitable accommodation. Providing false information creates difficulty during the rehoming process and may be detrimental to the cats well being. Friends United in Rescue, Inc is a 501(c)3 non profit organization that operates on available grants and donations. Unlike a municipal shelter, we do not receive any government funding. When surrendering a cat to our organization you must provide a surrender fee IF the cat is not up to date with vaccinations AND already spayed or neutered. The surrender fee for an unvaccinated and unaltered cat over 6 months old is $50. The surrender fee for an unvaccinated and unaltered cat under 6 months old is $10. If your cat has been spayed/neutered and vaccinated you must provide proof such as a spay/neuter certificate, vaccine record or veterinary invoice. Do you understand? *

If required to pay a surrender fee and once your request is approved, the surrender fee must be paid in full BEFORE your relinquishment appointment. Do you understand?

If your request is approved, you agree that you, your spouse and any other co-owners of the cat are irrevocably transferring and relinquishing the legal ownership of the cat to Friends United in Rescue, Inc. Do you understand? *

If your request is approved, Friends United in Rescue, Inc will maintain ownership of the cat until new, suitable living arrangements can be found. Friends United in Rescue, Inc is not liable to you for any losses, injuries, damages, costs, expenses, or other liabilities sustained or incurred by you in connection with your surrender, and or subsequent adoption, foster or other arrangements made for the cat. Friends United in Rescue, Inc makes no guarantees that the cat will be available if you choose to want the cat back. If the cat is available and Friends United in Rescue, Inc agrees to return the animal to you, you will be responsible for reimbursing Friends United in Rescue, Inc for all costs incurred by caring for the cat. This form contains the entire agreement between you and Friends United in Rescue, Inc. No modifications of this agreement will be valid unless in writing and signed by you and a Friends United in Rescue, Inc representative. Do you understand? *
