Intake Form - Small Animals

The aim of the Small Animal intake form is to ensure we gather as much information as possible about the animal requiring space in our care

Once this form is submitted, your request will be reviewed by our Care Team and added to our waitlist. Please note that our waitlist is extensive, and as part of the RSPCA, we must prioritise emergency intakes referred by RSPCA Inspectors. To help us respond as efficiently as possible, please ensure all information provided is detailed, honest, and accurate. Due to the high volume of requests, if you have not received a response within 3 weeks of submitting your form, it is unlikely that we are able to assist with your situation at this time.
Things to know: You’ll be asked to share details of medical history, behaviour, and photos of your animal. If you're looking to rehome more than 1 animal then you will be required to fill out the form for each individual animal. It takes around 10-20 minutes to complete an application for each animal. *	Euthanasia Policy* We will never put an animal to sleep to make space for another animal to come into us. Pets who's emotional and physical needs cannot be safeguarded will be assessed, reviewed and necessary steps implemented to improve this. If a pet is found to have an untreatable or serious medical problem(s), or behavioural issues so severe we deem them unsafe to be rehomed, then euthanasia will be considered.
