I am the property owner or am acting on behalf of the property owner with written permission to trap, seek spay/neuter, rabies vaccine, and release community cats via the Animal Shelter of Sullivan County (ASoSC) Community Cat program.

This program is for those outside cats that do not necessarily belong to you or anyone else. The process for these community cats includes an ear tip when getting altered. If you do not wish for the cat to have an ear tip (identifies it easily as a cat that has been fixed), please complete the owned animal spay/neuter form. I am requesting help from ASoSC to spay/neuter and provide rabies vaccine for community cats living on approved property. I understand there is a charge of $20 per cat. I will notify the shelter if I need assistance.

Any community cats or kittens trapped on my property that have the potential to be adopted can only be placed for adoption at the ASoSC with the approval from management.

For elderly/disabled individuals only, ASoSC has permission to trap until the entire colony has been trapped.

I will cooperate with ASoSC fully in this endeavor and understand that failure to pick up cats after alteration will result in fines to be assessed per cat abandoned in addition to per mile charge for transport back to the colony for them to be re-released.

Veterinarians affiliated with ASoSC shall Spay or Neuter approved community cats, vaccinate the cats for rabies, and ear tip the cats for identification. If a cat has major medical issues, the veterinarian will discuss with ASoSC the best options for that cat which may include euthanasia for the health of the colony.

I certify that I am the caregiver of these cats but do not take ownership of the cats in my colony.

I agree, as the colony manager/caretaker, to limit my feeding times to no more than one 30 minute window per day for the community, monitor the cats for overall physical conditions, be alert for any new un-altered cats joining the colony, and notify ASoSC that trapping and spay/neutering of the new cat is needed prior to trapping.

I understand a trap deposit of $50 is required if I rent a trap from ASoSC to trap cats in my colony.

All cats coming for surgery MUST BE IN A TRAP. NO CARRIERS ACCEPTED. One cat per trap. Put last name on trap with tape. All traps need newspaper or pad of some sort under the cats. If you do not have a trap, you will be given one upon arrival to move the cat over into.
