If you need to surrender or rehome more than one animal please complete a separate form for each animal. A SURRENDER FEE OF $200 PER DOG MAY APPLY IF THE DOG IS PLACED INTO RESCUE'S CARE TO ASSIST WITH TRANSPORT AND VET COSTS THAT MAY BE INCURRED.
