By signing below, I, the undersigned, acknowledge and understand the following terms related to the Trap-Neuter-Return (TNR) Program:
Rabies Vaccination Requirement:
As per state law, all cats in the TNR program will receive a rabies vaccination. This is a state-mandated requirement to ensure public health and safety. I understand and consent to this vaccination.
Ear Tipping:
I understand that, as part of the TNR procedure, my cat will undergo a routine ear tip. This is a standard, humane procedure used to indicate that the cat has been spayed or neutered and will help prevent unnecessary trapping in the future.
Health Risks and Possible Complications:
While the TNR program is designed to be a safe and effective procedure, I acknowledge that, as with any medical procedure, there is a small risk involved. In rare instances, some cats may experience complications that could result in harm or even death. I fully understand this risk and accept responsibility for the well-being of my cat during the procedure.
Liability Waiver:
By signing this document, I waive any and all claims against the TNR Program, the animal shelter, its staff, volunteers, and affiliates for any injury, illness, or death that may occur to my cat during or as a result of the TNR procedure. I understand that every precaution will be taken to ensure my cat’s safety and well-being. I also understand bringing my cat in a trap or hard sided carrier is MANDATORY and I will be turned away without proper containment. Neither the shelter, nor the vet will be held liable should my cat be improperly contained.
Pick-Up:
I agree to pick up my cat at the designated time and location as instructed. If I fail to do so, I understand that I may incur additional fees or the cat may be rehomed or placed under the care of the shelter at its discretion.
