By signing below, I, the undersigned, acknowledge and understand the following terms related to the Spay and Neuter Program:
Rabies Vaccination Requirement:
I understand vaccination against rabies is a state requirement. If I fail to provide proof my animal was vaccinated they will receive one day of surgery at an additional fee.
Microchipping:
I understand that, as part of this program my animal will be microchipped at no additional cost and unless my animal is already microchipped, this process is mandatory.
Health Risks and Possible Complications:
While the spay/neuter process is designed to be a safe and effective procedure, I acknowledge that, as with any medical procedure, there is a small risk involved. In rare instances, some animals may experience complications that could result in harm or even death. I fully understand this risk and accept responsibility for the well-being of my animal during the procedure.
Liability Waiver:
By signing this document, I waive any and all claims against this program, the animal shelter, its staff, volunteers, and affiliates for any injury, illness, or death that may occur to my animal during or as a result of the spay/neuter procedure. I understand that every precaution will be taken to ensure my animal’s safety and well-being. I also understand punctuation is important and if late, I forfeit all rights to a refund.
