You are bringing a stray animal to the Humane Society of Barron County, Inc. There are several points about which we would like to inform you. Please check each box after reading each statement to ensure us that you have read and understand the stray surrender statement.

HSBC does not guarantee that all surrendered animals will be made available for adoption.

If an animal is made available for adoption, there is no guarantee that the animal will be placed in a new and permanent home.

HSBC has the right to advertise this animal, using their name, age, and breed description on local radio stations, area newspapers, and social media.
You are relinquishing all rights to ownership of the animal. In signing you authorize HSBC to make the final decision on the disposition of the animal, whether it be adoption or transfer to another shelter/rescue group.

If you would like to adopt the animal later on, you will follow the same adoption procedures as any potential adopter. After the adoption application is filled out, the shelter staff will determine if the placement of said pet is appropriate. The adoption fee will be paid and said animal will be spayed or neutered in accordance with shelter policy.
I have read and understand the terms of this stray surrender statement. I understand that the HSBC will become the sole owner of the animal(s) and may care for the animal(s) as HSBC sees fit. To the best of my knowledge this animal has not bitten anyone.
