Adoption Application - Dog

Thank you for applying to adopt a dog from Vada’s Angels Animal Rescue. Please answer every question honestly and give as much detail as possible. Your answers help us assess your household, understand what you are looking for and match you with a dog whose needs suit your lifestyle.

After we receive your application, a volunteer will review it and may contact you for more information, a phone interview and a property assessment. Submitting an application does not guarantee approval or the placement of a particular dog. Vada’s Angels is operated by volunteers, and we appreciate your patience while your application is reviewed.

Applicant Declaration

By submitting and signing this application, I confirm that:

  • The information I have provided is complete, accurate and not misleading.
  • I consent to Vada’s Angels Animal Rescue completing reference checks, a phone interview and a property assessment as part of the application process.
  • I understand that submitting an application does not guarantee approval or the placement of a particular dog.
  • If approved, I may be required to complete an adoption trial of up to two weeks. Vada’s Angels retains legal ownership of the dog throughout the trial period.
  • I will keep the dog on a long line whenever outside during the trial period, unless Vada’s Angels provides different written instructions.
  • I will contact Vada’s Angels immediately if the dog requires medical attention or if there are any concerns involving the dog’s health, behaviour, safety, welfare, escape risk or care.
  • I understand that the applicable adoption fee must be paid before the dog is handed over.
  • If the adoption does not proceed, the adoption fee will be refunded less the approved $50 administration fee.
  • If I can no longer care for the dog, I will contact Vada’s Angels. I will not privately sell, give away, transfer or rehome the dog.
  • If Vada’s Angels has serious concerns about the dog’s care, health, safety or welfare, Vada’s Angels may require the dog to be returned while the situation is reviewed or investigated.
  • If the adoption proceeds, I will complete all required adoption, ownership, microchip and council-transfer documentation.
  • I will keep the dog as a domestic companion. The dog will live indoors or in an outdoor area specifically assessed and approved by Vada’s Angels.
  • Neither I nor any member of my household has been subject to a formal complaint, investigation or legal action relating to animal cruelty or neglect, or had an animal removed for breaching animal-welfare requirements, except where full details have been disclosed in this application.
  • I understand that caring for a dog involves risk. To the extent permitted by law, Vada’s Angels is not responsible for damage caused by the dog or for costs I incur.
