*	I understand I will need to maintain constant communication with the team at the LPCOAC / JBAAC in regard to the health, growth, and development of the foster(s).
*	I ensure that the dog / puppy lives primarily indoors and is NOT left outside unattended, even in a fenced yard. If the foster parent does not have a fenced yard, a dog / puppy will be walked on a leash on a regular basis. Cats are NOT to be allowed outside under any circumstances while being fostered.
*	I agree to assume all responsibility for damage caused by the animal either directly or indirectly while in my home.
*	I understand LPCOAC / JBAAC will take every precaution necessary to ensure the animal(s) are in reasonably good health.
*	I understand that any health issues must be reported to the LPCOAC / JBAAC. Vetting (emergency or otherwise) obtained without consent will be the responsibility of the foster parent.
*	I understand that transportation for booster vaccines, vet visits, picking up new supplies, medications if needed, etc. as needed.
*	I understand it may be necessary for the animal to temporarily return to the shelter for meet & greets, vet appointments, etc.
FOSTER WAIVER OF LIABILITY ** I am aware that during foster service at the La Porte County Small Animal Shelter, I could be injured or bitten. I hereby release the La Porte County Small Animal Shelter from any and all liability connected with my duties as a foster. *	NoYes
*** By submitting this, I agree the information I have given in this application for a Foster animal is true and correct to the best of my knowledge, and that any misrepresentation of the facts may result in my being denied the privilege of fostering a pet. I give The Jane Bernard Animal Adoption Center permission to contact the veterinarian and landlord I have listed for the purpose of verifying information. I understand that The Jane Bernard Animal Adoption Center had the right to approve or deny this application, in accordance with its policies established by the Board of Directors.
